Monitoring routines need clear documentation to survive staff changes and system updates. This template documents repeatable audit analytics, thresholds, ownership and escalation for each monitoring test.
Use it when turning an audit analytic into a recurring routine. The header records monitoring theme, data source, frequency, owner, first run and review forum. Part 1 covers test logic: risk statement, population and fields, and rule or query logic, followed by thresholds: alert threshold, materiality or tolerance, and false-positive handling. Part 2 covers the response: who reviews alerts, evidence retained, and escalation and trend reporting. A working table lists each test with data source, frequency, threshold and response owner.
The worked example is a monthly duplicate supplier invoice test using vendor, invoice number and amount with a fuzzy date window, where all exact duplicates and the top fuzzy matches are investigated. The completion page provides a final quality check, sign-off, final decision and outstanding follow-up, so every routine has defined logic, an owner and an escalation route.
